The below dashboards cover various Continuum of Care performance measures from January – December 2018. The data comes from the Homeless Management Information System (HMIS) and reflects the performance of The Way Home’s permanent housing programs (Permanent Supportive Housing [PSH] and Rapid Re-housing [RRH]) and Coordinated Access.

The embedded dashboards below are interactive and have the ability to be filtered in a number of different ways. The graphs below are not full size – to view the graphs in full screen, click the square in the bottom right-hand side of each graph. Click escape to exit full-screen. Graphs cannot be viewed full screen from some mobile devices and tablets, but mobile device viewers can view all of the data by scrolling left to right.

Coordinated Access Assessments

This graph shows the number of individuals assessed for housing each month and organized by population types (families, single adults, young adults).

Permanent Supportive Housing (PSH) Waitlist History

The number of people experiencing chronic homelessness on the waiting list for PSH each month organized by population type (families, single adults, young adults).

Rapid Re-housing (RRH) Waitlist History

The number of people experiencing literal homelessness on the waiting list for RRH each month organized by population type (families, single adults, young adults).

Median Days on PSH Waitlist

The median number of days people spend on the waiting list for PSH each month. These graphs cover the time from assessment until their referral to a program and shows both the overall wait as well as the wait by population type.

Median Days on RRH Waitlist

The median number of days people spend on the waiting list for RRH each month. These graphs cover the time from assessment until their referral to a program and shows both the overall wait as well as the wait by population type.

Coordinated Access Referral Outcomes

This graph shows the outcome of each referral sent to PSH and RRH programs for the month when the outcomes were recorded in HMIS. This graph is able to be filtered by permanent housing type (PSH or RRH), by provider, and by outcome type.

Coordinated Access Referral Denials

This graph shows the number of people not accepted into a permanent housing program in the month when the referral was closed. The graph is organized by the reason for each denial and can be sorted by permanent housing type, provider, and reason for the denial.

Permanent Housing Placements

This graph shows the number of people who moved into a permanent housing unit each month. The graph can be filtered by household type, chronically homeless status, Veteran status, provider, and permanent housing type.

PSH Project Size and Retention

This graph shows the capacity of each PSH program as well as the percentage of people who remained in the program for at least 12 months. This graph can be filtered by provider.

RRH 12 Month Exits

This graph shows the number of households that have exited from the RRH program after 12 months and whether the exit was to permanent housing (a positive exit) or not to permanent housing (a negative exit). This graph can be filtered by exit type and by provider.

PSH 12 Month Returns to Homelessness

This graph shows how many households returned to homelessness in the 12 months after they exited a PSH program. The date range for PSH exits are also during a 12 month period, meaning this data set could potentially cover 24 months. This graph can be filtered by provider.

RRH 12 Month Returns to Homelessness

This graph shows how many households returned to homelessness in the 12 months after they exited a RRH program. The date range for RRH exits are also during a 12 month period, meaning this data set could potentially cover 24 months. This graph can be filtered by provider.
